PADI FORMS
Below are Standard Forms for the respective training agency. Select the form(s) you were instructed to complete and supply to your training organizer.
Some forms may be filled out by viewing them within a PDF Browser then printing out the form. Some forms need to be printed to be filled out.
If under the age of 18, a parent or guardian must date and sign the form in addition to the student.
On the Medical questionnaire please answer the questions on your past or present medical history by spelling the word YES or NO. If you are not sure,
answer YES or checking the respective box.
As a side note, if you travel to destinations to dive, it is becoming more common for dive establishments around the world to ask to see a copy of the Medical Questionnaire. Medical questionnaires are good for 1 year from the date they are signed. A lot of sites do not allow the use of gloves. If you can show a medical need for gloves, have your physician note this on the form.

FEEDBACK PROTOCOL ON 2020 DIVER MEDICAL
In 2020, the international Diver Medical Screen Committee (DMSC) released a new diver medical screening system and guidance to the physician, which is being used for PADI courses and programs worldwide. - Form 10348 Florida Minor Child Parent Agreement* (When conducting any program in Florida, USA only) you must have parent/guardian also sign the Florida Addendum Notice anytime children will sign a release to participate in training and non-training activities. This is required whether you are operating as a Dive Center/Resort or as an individual PADI member.
Dive Operators
Dive and Travel Operators may have requirements for you where one of these is to complete and provide them with the respective form(s). Below are common disclosures and forms you may be asked to complete and submit prior to any activities.

$248 initial installment for instructors
