Below are Standard Forms for the respective training agency. Select the form(s) you were instructed to complete and supply to your training organizer. <\/p>\n\n\n\n

Some forms may be filled out by viewing them within a PDF Browser then printing out the form. Some forms need to be printed to be filled out. <\/p>\n\n\n\n

If under the age of 18, a parent or guardian must date and sign the form in addition to the student.<\/p>\n\n\n\n

On the Medical questionnaire please answer the questions on your past or present medical history by spelling the word YES or NO. If you are not sure,
answer YES or checking the respective box.<\/p>\n\n\n\n

As a side note, if you travel to destinations to dive, it is becoming more common for dive establishments around the world to ask to see a copy of the Medical Questionnaire. Medical questionnaires are good for 1 year from the date they are signed. A lot of sites do not allow the use of gloves. If you can show a medical need for gloves, have your physician note this on the form.<\/p>\n\n\n\n
